Gaia Online, FlyFF, Adventure Quest, Mechquest, hell even Furcadia all began with a concept and a lone programmer. Even some games that weren't created independently at first have ended up as indie games, like Subspace/Continuum.
What does it take to get a project rolling, though? Talent alone, or just the desire?

What does it take? Everything. Your heart and soul must be in the game for you to enjoy making it. Now to be successful at it is a whole different game.

That depends on a lot of things. Single player or multi player? Those are entirely different areas.
http://www.sirlin.net has some good articles for competitive multiplayer games. Don't listen to his single player stuff though, it's stupid.

saying make it addictive is like saying coca cola has flavor.
but what about it makes it addictive is the bigger question.
i myself is in the design phases for a small morpg, (notice i didnt include the first m, i have no illusions that it will be a massive game, just something fun for me and clan buddies, and if it somehow magically took off, then great)
i'm an average programmer myself, i know the basics of c++ and java, having a year of each in high school, but i also have the desire, which i think accounts for more. too many times i've strolled through forums of people sayign they have a great idea for an mmorpg, but they dont know what to do, and out of that they expect a dev team.
where there is a will, there is a way. and i believe that especially holds true for game development. look at what current games thrive on, study the trends, and if your smart you'll try and incorporate some of that into your game, all while adding your own personal flare.
